Improving Access to Integrated Remote Health and Social Care

Funded by a grant from the Mother Cabrini Health Foundation (MCHF), this project aimed to address deferred care needs for those underserved, underinsured, and uninsured in Queens and the Bronx - the New York City areas hardest-hit by the COVID-19 pandemic. This was achieved by connecting high-risk patients with AIRnyc’s community health workers (CHWs) who addressed their social needs and connected them to providers that can address care caps. The project assisted people who may have avoided the health care setting during the height of the pandemic, and who did not subsequently return to care.
