GNYHA was awarded a grant from the New York State Health Foundation for a two-year project starting on October 1st to focus on improving the integration of primary care residents with community based organizations (CBOs) in New York State. A primary goal of the project is to increase resident participation in the Delivery System Reform Incentive Payment (DSRIP) program and population health activities. The project builds on prior work done by GNYHA aimed at improving care coordination and resident learning in outpatient settings, which was also supported by the New York State Health Foundation and engaged 15 residency programs statewide.
