GNYHA Foundation Receives Phase Two Award from CMS

On March 24, 2016, the Centers for Medicare & Medicaid Services (CMS) provided the GNYHA Foundation a Phase Two award under the CMS Initiative to Reduce Avoidable Hospitalizations among Nursing Facility Residents. CMS extended the project through October 2020 and added a payment reform component to the existing project activities. GNYHA Foundation’s New York–Reducing Avoidable Hospitalizations (NY–RAH) project will continue to improve patient care and the health and well-being of nursing facility residents. It also complements the work being conducted with nursing facilities as part of New York State’s Delivery System Reform Incentive Payment (DSRIP) program. Participation in the CMS initiative is available in New York exclusively through NY–RAH.

This award is a continuation of a four-year cooperative agreement that the GNYHA Foundation received in 2012 to conduct a special project to improve the quality of care for long-stay nursing facility residents.
